New Patients
For all new patients coming to our office please follow the guidelines below:
- Understand your insurance coverage for office visits and any procedures that you are anticipating in the course of your care with our physicians.
- Review our insurance and billing policies.
- Complete our New Patient Forms.
- Bring your driver’s license and your primary and secondary insurance cards.
- Bring with you all of your current prescription medications.
Returning Patients
During your visit we will collect as much information as possible from your primary physician. However, please bring any recent medical records with you at the time of your visit. Also, it is important that you bring your current insurance cards, driver’s license. We do require that all patients update their demographic information once a year in order for our physicians and staff to reach patients with important information with regard to testing results.
Office Hours
The Office is open between the hours of 9:00 am and 5:00 pm Monday through Friday (excluding holidays) and closed on the weekends.
Prescription Refills
The best way to refill a prescription is to contact your pharmacy directly. In most cases the process is automated and only requires the information printed on the label of your prescription bottle. If there no refills on the prescription, your pharmacy can contact our office to renew your prescription. Another way to refill a prescription is to contact our office at 845-471-9410 ext. 16. You will need the following information when refilling a prescription: Name, Date of Birth, Medication, Dosage, Pharmacy Name, Address and Phone Number. A third way to refill your prescription is by filling out the prescription refill form. If additional information is needed, a staff member from our office will contact you to discuss the need for additional medication or arrange an office visit to ensure proper treatment.
If the medication needs prior authorization you need to allow five business days for the office to obtain the authorization.
Office Policy is that if you have not been seen for over a year an office visit is necessary in order for us to refill a prescription. Also, prescriptions for Narcotics and Pain Medications will be filled during office hours and please specify if you would like to pick up the prescription or have it mailed to you. THE PRESCRIPTION CANNOT BE CALLED INTO THE PHARMACY.
Request for Medical Records
All patients requesting to have their medical records released to another hospital or physician office must fill out and sign our medical release form. Please allow 7 to 10 business days for this process
Referring Physicians
In order to make the process for our referring physicians easier, you may download the referring physician form. Please fax the form to 845-471-7943 to expedite the process of contacting patients for appointments.
